A fundamental breakdown of pilot discipline, including personal conversation in the cockpit, apparently was the major reason a US Airways commuter plane ran off the end of a West Virginia runway last month, according to industry officials.

Now, three weeks after the nonfatal incident, the circumstances are prompting extensive discussion and concern among pilots and safety experts, who consider it the latest example of cockpit distraction and erosion of pilot discipline.



Ouch....this one could easily have been much worse.

Without the engineered concrete it most likely would have been very much worse. That's not a place you want to run off the end of the runway. And some dismissed the idea that it was newsworthy when the story broke...

There are places where a runway excursion involves little more than muddy landing gear and a lot of paperwork, and places where it can lead to something very serious indeed. (LGA, YYZ, CRW, GUA, Funchal (can't think of the code). The collapsing concrete is needed at all those places. Goof think it did its thing here.

Why was this moved? Not only is PSA a US Airways Express carrier, it's a wholly owned subsidiary of US Airways Group, so has no relation to any airline other than US Airways?

It's almost like someone is trying to kill the board...

The Captain has been with the company for at least 8 or 9 years and upgraded 4 or 5 years ago.

No idea about the FO but due to furloughs, there aren't any FOs at PSA with less than 2 years time on the CRJ.

And it is a huge disservice to move this out of the Airways forum. PSA and PDT, like it or not, are part of US Airways Group.
