Pilot Fatigue

In light of potential duty day rule changes I'm doing a survey on pilot fatigue for my master's thesis. It is completely anonymous and takes about 2 minutes to complete. I appreciate any one interested in this to completing one. If requested I can post the survey results once I have compiled all the data. The link is below.

I have posted this on other forums like apc. I would like to edit the title of this so that it reads pilot fatigue survey, thus avoiding wasting peoples time that click on it only to realize that it doesn't apply to them, but I can't seem to edit the post.
