Was there any one intel/command failure which lead to 4000 deaths?... The insurgency wasn't something that anyone had planned or trained for, but that's been addressed.
 

 
Well yes there was.  The whole pretext to the war, weapons of mass destruction.  That one intel/command failure is what got the ball rolling.  Without that there would have been no war.

Which in book is a bigger story?  An intel/command failure the led to the death of four Americans or an intel/command failure that leed to the death of 4000?  A failure that buy the way is still having repercussions to this day.  Google Iraq and see what you get.  Maybe that's why you get the impression that more attention was focused by the "media" on Iraq.
