Not sure what's happening in the industry the last two weeks?? This is crazy all these accidents, I just heard a rumour a 205 crashed north of Terrace?? Anybody hear anything about it?
 
Ya heard the same thing, but is not a rumer. Ka 32 slung it out of the bush yesterday.All okay with crew I believe.Happened around Galore Creek area. Heard it was weather related.Belongs to a company out of Terrace.Wont mention any names just yet,I just dont do that even though I know who it is.

here is another prime example as to why it would be nice to have a preliminary reporting system in place much like the FAA has. Not to lay blame or point fingers, but simply to inform the general flying public that this kind of stuff is happening. A brief overview of what happened may give people enough insight that they too better keep on their toes or else they may find themselves in a similiar situation.
It's kind of like photo radar in a way, weeks later you get a ticket in the mail. How is that supposed to prevent speeding at the time?
Related to our industry, how is an accident report 2 to 3 years old when it's released, supposed to help prevent accidents or at least knock the situational awareness back up to the top where it should be?
 
Sounds like a good plan to me. It sure couldent hurt to have something in place like that would be very inforative.

I agree MagSeal. The Canadian Forces has a similar system in place and it works very well. After an accident/incident Flight Safety issues an initial report to the flying communities as soon as possible so that everyone can increase their vigilance. Its amazing what it can do to help you "readjust" your flying habits after you read one. Supplemental reports are issued as key info comes available and then it finally gets published in a summary once the event is wrapped up and the findings have been made. I know from my experience this summer, after a company lost a machine during refueling I had a hard look at my personal technique; however, the only way I heard about this incident was through the grapevine...sometimes effective but certainly not ideal.

I'm not sure how the FAA does it...there needs to be a certain degree of openess and I think most operators would be hesitant to contribute meaningfully to the system and subsequently put a spotlight on their operations...no matter how squeeky clean they are. I could be wrong. That's not saying it can't be done...
 
I would hope that the companies name could be left off the initial report, but I think that would seem pointless as it's pretty likely your customer base has already found out about it. Either they were involved, or you had to tell them their job has been cancelled due to a "lack of available equipment"
Just the direct facts reported would be a huge help to everybody. Leave out the speculation, leave out the finger pointing.

I feel that lobbying the TSB would be a start, but it'll take more than a few of us to accomplish the feat. They have their system, and likely aren't interested in changing it yet.

when a big airliner crashes they aren't shy about stepping into the lights and giving their preliminary reports to the media then, so why hold back with the rest? Are they afraid of legal issues?
 
Gentlemen Transport does have this product available, it is called the Transportaion Safety Board of Canada Daily Notification Log, Mode: AIR, Unfortunately it only goes out to a select few including insurance companies, and major airlines.

I recall on a previous accident thread, several months ago, someone posted these prelim reports on two accidents - they were to the point and useful and ended speculation. If that poster is still around, further "anonymous" posts of that nature would sure be well received.

(we promise not even to speculate on who or why, will just be pleased with the information)

And yes, I did hear that the Terrace operator mentioned was picking up a 205 to do some work around Eskay Creek.
