It's not hard to figure out CSAR, there's this little thing called a SCAB list, giving you and all your little SCAB buddies there the recognition you and they deserve. You can find the one for your neck of the woods at http://amfa5.org/InsertScabs/Scabs.php

My bet is that Frank Blazik is actually Frank Blazie. In any case, I think either AMFA Local 5 or Local 5 Striker has the name misspelled.
