Inadvertent Slide Deployments

There were two more ISDs recently on PHL based crews I believe.
I'm curious about the 737 1L ISD that injured the gate agent.
Do any FAs know what discipline if any was handed out to the FA who sent the GA to the hospital?
We all recently received the memo stating minumum 3days off w/o pay would be the starting point.

Are you accusing someone of a deliberate act? Perhaps you can find a better way to say that.

I am surprised there are not more slide deployments, with some of the most dinosaur-like "procedures" that conflict with nearly every human interface guideline associated with error-checking. Hint: try designing a checklist that fails gracefully and not catastrophically, like the checklists Tempe comes up with.

Actually, considering the number of slides that do not automatically inflate, I am surprised appropriate management is not given three days off w/o pay for each instance. Parker? Kirby?
 
For HP at least its usually catering that blows slides on the airbuses, its rare to see one blown on the 737/757's for some reason. Its usually not the fa's fault.

While the new monitor and challenge procedure doesn't really bother me that much it certainly has it's problems. There is WAAAAAAY to much crisscrossing of duties. The "A" should block the aisle and let the "C" do the front doors for BOTH departure and arrival. The "C" is on the floor doing the door with cords stretched over their heads and most don't know what they are saying. For flight attendants that ARE responsible for a deployment they should have focused on their door. Plain and simple. Sure situations come up and the conditions are not always perfect BUT the severity of a blown slide physically and personally(time off) is pretty BIG. Even with the old door arming/disarming procedure, if you paid attention and focused there was no reason to NOT arm or disarm the damn door. Get off the cell phone, stop chatting about your overnight, boyfriend/girlfriend, hot passenger in 1D and do the door. THAT is the problem MOST of the time. TEMPE or whomever was responsible for the new procedure being implemented can't even do THAT right. The little gray books printed the first time are WRONG. My god wasn't this AWESOME system in place out west for a few years. Can we say screw up a one car funeral. 🙄

Basically taking a jab at Tempe, they seem to be good as passing the blame around. They also seem to never admit they are wrong at alot of things as well. Tempe is very good at one thing, sitting on their arses and screwing things up. They would screw up a one car funeral.

Do you know if the west A320s have a hole for a pin to go in the slide disarming handle, when it is disarmed. East A/C have it, but the pins have been removed. I was told that the west A/C don't have it, but not sure if they meant the pin or a hole for it. Thanks

The West A319/320s do not have slide pin access without pyhsically removing the slide bustle.

It is a "FACT" that the majority of PHXs ISDs are caused by catering "NOT" the FAs or GAs.

First of all, the "C" would not have a cord around their neck. The interphone will be passed to the "A" after the doors are disarmed. Second: Nobody ever thinks they will blow a slide, no matter how much they think they are focusing. No slides have been blown by someone on their phone, or at least admittedly.

So, you have a totally different situation (slide pin access) and yet you morons continue to act as if the situations were the same and the FA is at fault, demonstrating a complete lack of knowledge and basic ignorance of situational awareness.

Every single FA trainer at PHX should be fired, no exceptions. The head of training should be fired. Heck, the CEO should be fired.

What is wrong with you people? Why are you so stupid you cannot figure out what is going on?

From now on, every push will involve me inspecting every door, every arrival will involve me. I will not subject any crewmember to such idiocy and cretinous behavior on the part of the morons at Tempe, may they burn in hell forever.

Not sure if you are east or west, but hopefully you are not east because if you are you should know that all of our slides automatically inflate when the door is armed and opened. It is simply our procedure to pull the inflation handle.

Don't blame the training department, talk to Policies and Procedures first.
