The Fokker , flight 405 LGA-CLE actually had a 2nd deicing, to this day, I cant explain what made me pause and watch when this . I watched it taxi out, shortly after that I jumped in a van with some other ramp guys trying to pull people out of the burning remains of an aircraft. sights, sounds and smells I will never forget. Some ramp guys from the US shuttle and Northwest were there with us before the CFR trucks arrived.

In addition to the ice buildup on the wings, there was so much wet snow/slush on the runway, we couldnt believe it. later speaking to the lead investgator for the NTSB, I brought this up. He casually dismissed it. hmm

The report said the crew rotated 11kt early , the aircraft lifted off, but stalled and cartwheeled as the wing hit the runway. It ended up partially submerged

USAir also lost another B732 - not very well publicized. at ISO, N210US went off the runway during takeoff due to a mechanical problem. The aircraft was a write-off
