U.S. Airways officials: Plane lost wheel prior to take-off

A U.S. Airways plane lost one of its wheels while taxiing for take-off Wednesday afternoon at Charlotte-Douglas International Airport, officials for the airline said.

That would be torque, not torch, lol.

How do you what kind of error?

The article didnt state it, it could have broke and not been human error.

Probably just some coffee and soda spilled on the nuts and bolts. Damned passengers. :D
 
Did the wheel actually come off, or is this more journalist malpractice--the tire blew?

"One of the four main tires separated from the plane while it was moving on the runway, rolled across the tarmac and lodged into the undercarriage of a US Airways Express plane that had just landed from Lexington, Ky."
 
Wheel coming off completely? Lets see what actually did happen.
Hard to believe that the wheel just loosened up and rolled off the axle.
 
The axle broke on that F100, there was an AD put out after that for the cross time bolts on the main landing gear, they had to be inspected daily too.
