New Jumpseat Form

I've had several friends tell me about a new USAirways Jumpseat Form. I've looked through the FOM and checked my CBS messages and haven't seen any revisions or notices. I've also had a couple of Jumpseaters in the last couple of days and they had the old form. They didn't know about any new form or policy. I guess I'll have to give the Chief Pilot a call Monday and see what's going on.

In the meantime could someone enlighten me, I would hate to deny someone a jumpseat because they didn't have the correct form.

Who cares, if there listed in the FOM give them a ride!!!
 
:cop: The topic of this thread is jumpseat forms. Stick to it. There is a thread for pilot labor issues. If you have comments for that topic, post them there. Do not hijack another thread.
 
Management doesn't follow the contract regarding "jump seaters" in most hub cities anyway, especially with downsize, overweight flights.
 
Why are some pilots denying other pilots jumpseat? Power play? Pitiful.

Yes there is a revision to the jumpseat travel request form. The changes are mainly to the top of the form which is for the gate agents. The layout for information is slightly different and there is no longer a blue copy.

The new form in question lists a code of conduct, ie: no bags in cockpit, talking (speak only when spoken to), etc. Also for the jumpseating pilot to initial a section regarding the election of USAPA and comments about their wisdom in tossing ALPA out.

I hope there isn't any none operational conversations going on during those long flights.
